fix(coderd/database): renumber migration to 000514 after rebase

Main advanced again with another 000513 (user AI budget overrides,
#25439) while this branch was open. Renumber the per-user MCP
custom_headers migration and its fixture to 000514 to avoid the
duplicate slot collision and let the new migration apply after main's
addition.
